  Clean criminal record certificate

• Second Stage:

  Copy of the "sketch" plan of the institution's website (a copy of the plot map)
  Location number (makani) / location coordinates (if applicable)
  Copy of the building plan showing the dimensions and sizes of rooms, halls, corridors and sanitary facilities

• Third Stage:

  Data of the center’s director and his academic qualifications
  Copy of experience certificates
  Copy of the title deed in case the building is owned by the license applicant or a copy of the lease contract
  Copy of Department of Economic Development license
  Copy of Civil Defense Department license
